It specifies when you eat, not what you eat, which is both its practical appeal and one of the reasons the evidence is complicated to interpret. Several distinct protocols get grouped under the same label, and their outcomes, adherence rates, and suitability for different populations vary considerably.
The mechanisms proposed to explain fasting's benefits include caloric restriction (most people eat less when their eating window is compressed), metabolic switching between glucose and fat oxidation, reductions in insulin exposure, and the cellular housekeeping process called autophagy. Which of these mechanisms drives observed effects in any given trial is usually not clear, which matters when you are trying to decide whether the protocol is right for you.
16:8 — The Most-Studied Protocol and What the Trials Show
Sixteen hours of fasting, eight hours of unrestricted eating, typically achieved by skipping breakfast or finishing dinner by early evening. This is the most studied variant of time-restricted eating, and what the human trials report on 16:8 time-restricted eating is modestly positive but far from dramatic.
Weight loss outcomes in controlled trials are generally comparable to daily caloric restriction. Some trials show improvements in cardiometabolic markers (fasting insulin, blood glucose, LDL cholesterol, blood pressure), while others show no significant difference from continuous caloric restriction. The most honest read of the literature is that 16:8 works primarily because it reduces overall calorie intake for most people, rather than through any unique metabolic mechanism beyond that.
The adherence profile is the protocol's real selling point. Many people find skipping breakfast considerably easier than counting calories throughout the day, and long-term adherence determines long-term outcomes. If a protocol you will actually maintain produces similar results to a theoretically superior one you will abandon, the pragmatic choice is clear.
5:2 — Two Low-Calorie Days vs Daily Restriction
The 5:2 method involves five days of unrestricted eating and two non-consecutive days of very low calorie intake, typically around 500 to 600 kilocalories on the fast days. The intermittency is the distinguishing feature, though the mechanism is almost certainly the caloric deficit produced across the week rather than anything unique to the fasting days themselves.
Trial evidence places 5:2 roughly on par with daily caloric restriction for weight loss at six to twelve months, with some trials showing slightly lower dropout rates, possibly because five days of normal eating provides psychological relief that sustained daily restriction does not. Cardiometabolic improvements in 5:2 trials generally mirror those seen in matched caloric restriction trials, which suggests the method works, but that there is nothing special about the specific format beyond what the deficit produces.
The main practical challenge is that the 500-600 kilocalorie fast days require planning. People who find hunger difficult to tolerate, or whose work or social life makes irregular food intake disruptive, often find 5:2 harder to sustain than 16:8.
OMAD — Simplicity with the Thinnest Evidence Base
One meal a day compresses the entire daily caloric intake into a single sitting, producing the longest daily fasting window of any mainstream protocol. The appeal is extreme simplicity: one decision about food per day. The evidence base is the thinnest of the three major protocols.
The controlled trial data on OMAD specifically is sparse. Most of what gets cited is extrapolated from alternate-day fasting research or very-low-calorie single-meal studies with small sample sizes. There are genuine safety considerations: consuming a full day's nutrition in a single meal can produce substantial post-meal glucose excursions, may be difficult to achieve with adequate protein for muscle protein synthesis, and is not appropriate for people with certain health conditions or disordered eating histories.
For most beginners, OMAD is not a sensible starting point.
Women's Hormone Considerations: What the Research Actually Says
This is an area where confident claims on both sides outrun the evidence. Fasting's effect on women's hormones and what the evidence says is a topic that deserves more careful research than it has received.
Some case reports and small studies have documented menstrual irregularities in women who adopted prolonged or aggressive fasting protocols, particularly those combined with significant caloric restriction and high exercise volume. The proposed mechanism involves the hypothalamic-pituitary-ovarian axis, which is sensitive to energy availability signals.
However, the controlled trial literature on moderate time-restricted eating in women does not consistently show hormonal disruption in healthy, well-nourished adults. The population matters: lean women with already limited energy reserves appear more vulnerable than women with higher body fat percentages who are reducing overall calorie intake intentionally. Blanket claims that intermittent fasting is dangerous for women's hormones, or conversely that it is uniformly safe, both exceed what the literature supports.
How to Pick the Right Starting Point
The evidence suggests starting with the most sustainable protocol rather than the most aggressive one. For most people that means 16:8, beginning with a compressed eating window of ten to twelve hours rather than immediately restricting to eight. Adjusting gradually allows the body to adapt and reveals whether the approach is compatible with your lifestyle before you commit.
Food quality within the eating window matters. Time-restricted eating does not neutralise a poor diet, and some trial participants respond to the compressed window by eating larger quantities of calorie-dense food, eliminating any deficit benefit.
When to Stop and Seek Professional Guidance
Fasting is not appropriate for everyone. Pregnant and breastfeeding women, people with a history of eating disorders, those managing type 1 diabetes or insulin-requiring type 2 diabetes, and people on medications that require food intake should not adopt any fasting protocol without medical supervision.
If you experience persistent fatigue, dizziness, significant mood disturbance, irregular menstrual cycles, or unexplained weight loss while fasting, those are reasons to stop the protocol and discuss symptoms with a GP or dietitian.
